Crazy

Some days I think we are all nut cases ..how could we not be ? medical is crazy..when I left on Friday at 330pm we had 2 nurses on for the evening shift..as one had called in sick...

we had 2 waiting to go to the 6th floor..god knows how many needing hoists..one gastro faecal incontinent everywhere..she has been changed 4-5 times that morning with showers according to W...and 4 new admissions 3 not yet seen or admitted ..a new admission just lying there not very responsive..C was like..lets call a met call so someone can come and help us..either that or we will ship him across the road..

he came in with dehydration..he was only discharged on Saturday...K was like..you would think that the nursing home would have given him a drink wouldn't you....anyone volunteering to stay behind?? the other nurses just laughed and shook their heads..it has been a hell of a day..

K continued...it's okie V will stay back and give us a hand..I told her you've got to be kidding..after the day we had this morning I am out of here..Out patients sounded like heaven compared to here..

It was a hell of a day..discharges flying everywhere..nearly forgot one..only to be reminded by C..lucky he was a pre-planned one thus all done anyway..just had to come up to talk to him..he was still waiting for his lift..the nicest bit was him asking me have I ever considered retail ? I told him no..been there done that have never taken to it..

then asked across the road to bring over antibiotics for me as someone was hanging in there by her bare threads...then her doctor came up and I had to run back down again to look up dosing regimen for him...

found about 5 going over the weekend thus had to prepare for them..one was already prepared only to find out at the last minute the covering doctor made an error on her chart for the district nurse..by the time I had found out he's already left  for the day..

so off I went to try and find the other rehab cover to amend it for me...for her renal function was okie so she didnot need the dose reduction for her Clexane..


rang rehab he has already gone..apparently he went up to level 4 surgical..so  I rang up to level 4..K please hold him for me don't let him leave..luckily he was still there and was very kind to change it to the correct dose for me for she wasn't really his patient..

after lunch had to rush across the road to pick up my DDs..as morphine was running low and with 2 palliatives it was not going to last them over the weekend..

by the time I had signed in the DDs it was 240pm..had just enough time to do the 3 newies one was sleeping but was under pain/surgical so I let him be..probably just antibiotics and pain killers....the unresponsive guy was only recently in so I checked his discharge profile and did owing scripts for the uncommon ones..the others they can use ward stock until his charts are written up..the third one had a medicopak and all tablets we had as ward stock anyway..

I ended up leaving at 330pm..couldn't do anymore..it was a hell of a day..learnt something though..learnt Timentin is given 8 hourly for Crcl 10-50 and given 12hourly for Crcl of less than 10.. that Clindamycin IV is given for Strep group A...and no need for dosing reduction in patients with declining renal function...her doctor was very obliging when I asked him his reasoning for using Clindamycin instead of Benzyl penicillin or Ampicillin since she wasn't allergic to penicillin..he told me he believed it is Strep toxic shock syndrome thus that was why he wanted Clindamycin..

funny that no matter how stressful it is we always come back for more..it is addictive..some days you want to chuck the towel in...then the next day it is not so bad and you think it is not so bad..and then it gets bad again..
